Form not in table T512P: VETA FORM: VETA

Hi Experts,

Client requirement is to use VET 100A report. I tried to run standard report S_L6B_69000188 when executed I got below error

Form not in table T512P: VETA FORM: VETA

When checked in T512P table there is no VETA form. I have downloaded the form and activated as per this below note

Note: 1719339 - VETS: Forms VETS-100 and VETS-100A for 2012  

when i check T512P i dont see VETA form. where can i download this form and how to populate it in T512P

we are on ECC 6
